Albany Gastroenterology Consultants Data Breach Settlement resolves claims through a $200,000 settlement without further litigation. Class members who received notice may be eligible to file.
The automatic 2-year CyEx Medical Shield Complete credit monitoring (with $1M medical identity theft insurance) requires no claim form. Separately, class members choose ONE of two cash options — reimbursement of up to $2,500 in documented out-of-pocket losses, or a no-proof alternate cash payment estimated at $10. Unlike some settlements, these two cash options do NOT share one pool: there is a $100,000 sub-cap for the documented-loss option and a SEPARATE $100,000 sub-cap for the no-proof option, so a surge in one option's claims cannot reduce payouts in the other. A rejected documented-loss claim that isn't cured is automatically converted to a no-proof claim.
How to File Your Claim
- Two years of CyEx Medical Shield Complete credit monitoring is AUTOMATIC — enrollment codes were already mailed, no claim form needed for that benefit (activates after final approval)
- For the separate cash payment, file online using the Login ID and PIN from your notice, or mail the paper form
- Cash claims due October 5, 2026
- Opt-out/objection deadline is six weeks earlier — August 21, 2026
