DAP Health Data Breach Class Action Settlement allows eligible class members to file claims from a $1,300,000 fund. Do I Qualify?
You may be eligible if:
- Your Private Information was compromised in the DAP Health Data Incident in July 2024
- You received notice of the incident, or you otherwise meet the nationwide class definition
- Eligible states: Nationwide (US)
- California residents as of July 22, 2024 who were sent notice may also claim the additional $75 statutory cash payment
- You are not an excluded judge, Class Counsel family member, DAP Health entity or officer, or a person who timely opted out
- You file a valid claim by October 21, 2026
The online claim portal requires the Unique ID and PIN printed on your notice. Documented-loss claims also require supporting documentation such as credit card statements, bank statements, invoices, telephone records, photographs, or receipts. A personal certification, declaration, or affidavit alone is not enough. If you cannot locate your Unique ID and PIN, request them through the contact page on the official settlement website, or download and mail the paper Claim Form.

The Riverside Superior Court preliminarily approved the settlement in Donald Crosslin and Matthew Paone v. DAP Health, Inc., Case No. CVRI2500186. The claim process is running through Epiq. The underlying event is a cybersecurity incident in July 2024 that resulted in the potential unauthorized access to or acquisition of class members' Private Information, including names, Social Security numbers, addresses, dates of birth, financial account numbers, Medicare and Medicaid numbers, health insurance numbers, and medical records. DAP Health, Inc. is a nonprofit community health provider headquartered in Palm Springs, California. DAP Health denies the legal claims and denies wrongdoing or liability.
Benefits stack rather than requiring a choice: estimated $25 pro rata cash, plus $75 for California residents as of July 22, 2024 who were sent notice, plus up to $5,000 documented losses, plus two years of credit and medical record monitoring. The Final Approval Hearing is October 1, 2026 at 8:30 a.m., before the October 21, 2026 claim deadline. No payment date had been announced as of August 18, 2026.
