- You are a veteran who was charged a fee by Just4Veterans LLC
- The fee was in connection with a VA disability claim
- The fee was charged on or after September 20, 2020
- Eligible states: Nationwide (US)
- You file a valid claim by October 5, 2026, unless you are seeking only forgiveness of an unpaid fee (no claim form required for unpaid-fee forgiveness)
