A targeted cyberattack discovered around January 25, 2024 potentially exposed names, addresses, dates of birth, Social Security numbers, driver's license numbers, financial account numbers, patient IDs, and medical, treatment, and insurance information for approximately 10,772 people.

How Much Will I Actually Get?
Choose one option: (A) up to $2,000 in documented ordinary expenses plus up to $5,000 in documented extraordinary losses plus up to $80 in lost time (4 hours at $20/hour) for up to $7,080 total, plus one year of free CyEx Medical Shield Total credit monitoring with $1 million insurance; OR (B) a flat $50 cash payment with no proof required, in place of monitoring and other benefits (not in addition). Claims-made uncapped per-claim structure.
Documented losses may receive higher compensation.
