- You received (or were entitled to) notice of the incident
- About 130,583 people were notified
- Eligible states: All US states
- You file a valid Claim Form online or by mail by October 19, 2026 using the LoginID and PIN from your notice
Online filing uses the LoginID and PIN from your notice. Choose documented losses up to $5,000 if you have receipts or bank records, or the $50 alternative cash payment with no documentation required. All class members may also enroll in two years of CyEx Medical Shield Complete (codes were mailed by postcard).

What Happened?
In December 2024, a targeted cyberattack on Community Dental Care systems allegedly accessed files that may have contained names, health insurance information, dates of birth, medical information, and Social Security numbers.
The case is In re Community Dental Care, Case No. 62-CV-25-2490 (District Court for Ramsey County, Minnesota). Community Dental Care denies wrongdoing.
The claim deadline is October 19, 2026. Opt-out and objection deadlines are September 24, 2026. A Final Approval Hearing is scheduled for November 23, 2026, at 11:00 a.m. Central Time.
