Frequently Asked Questions

Who qualifies for the MCNA Dental Data Breach Settlement?
You may qualify if you received notice that your private information was potentially impacted in the MCNA / Healthplex data incident between February 26, 2023 and March 7, 2023. Eligibility is nationwide and is based on receiving that notice, not on the state where you live.
What is covered by the MCNA Dental data breach settlement?
The settlement provides cash payments of up to $2,500 for documented out-of-pocket losses fairly traceable to the data incident, plus two years of medical data monitoring. Monitoring is available to settlement class members who do not opt out and does not require a claim form. Cash payments require a timely Claim Form with supporting documentation.
Is there a cap on documented losses in the MCNA settlement?
Yes. Documented out-of-pocket loss payments are capped at $2,500 per person, and the documented-loss fund itself is capped at $250,000. If valid claims exceed that fund, payments are pro-rated. Medical data monitoring is a separate benefit and is not paid from that $250,000 cap.
When can I enroll in MCNA medical data monitoring?
You do not need to file a Claim Form for monitoring. If you received notice by email or mail, it included a unique enrollment code. Codes become active for use within 30 days after the Effective Date of the settlement, which follows final court approval. Check the official settlement website for activation updates, or contact Kroll Settlement Administration LLC if you cannot find your code.
What is the MCNA Dental Data Breach Settlement deadline?
The deadline to submit a Claim Form for documented out-of-pocket losses is October 19, 2026. File online at mcnadatabreachsettlement.com or mail the form to Kroll Settlement Administration LLC by that date. Missing the deadline can forfeit the cash-payment option; monitoring does not require a claim form.
