What Happened?

HCF Management, Inc. notified current and former residents, patients, and employees that their personal information — including Social Security numbers and medical information — was compromised in a September 2024 data breach at its Ohio and Pennsylvania long-term care facilities.

The parties reached a class action settlement (Bruce, et al. v. HCF Management, Inc., et al.) with a claims-made structure and no published aggregate fund. Class members who received notice may choose one cash option and can also elect credit monitoring.

Cash option (a): a flat $35 payment with no documentation required. Cash option (b): up to $400 for documented ordinary out-of-pocket losses plus up to $4,000 for documented extraordinary losses (these stack). Cash option (c): up to 4 hours of lost time at $20/hour ($80 cap). All class members can also elect 3 years of one-bureau credit monitoring including $1M identity theft insurance, regardless of which cash option they choose.
