Memorial Health Services has agreed to settle a class action alleging its MyChart patient portal used tracking pixels that disclosed patient information to third parties without consent. If you accessed Memorial Health MyChart between March and July 2022, you may receive a pro rata cash payment. Deadline: August 21, 2026.

You may be eligible if:
- You accessed the Memorial Health Services MyChart patient portal between March 7, 2022 and July 8, 2022
- You received a settlement notice with a Login ID and PIN
- You submit a valid claim online or by mail before August 21, 2026
- You do not need to provide MyChart login credentials or medical records to file
