A data security incident occurred at Total Vision LLC on October 30, 2020. Plaintiffs allege the California vision and eye-care group failed to adequately protect patients' personal and confidential medical information, in violation of the California Confidentiality of Medical Information Act and other state laws.

How Much Will I Actually Get?
A pro-rata cash payment from the Net Settlement Fund with no proof needed beyond contact information, PLUS optionally up to $1,000 for documented out-of-pocket losses fairly traceable to the breach (documentation required, mail-only for the documented-loss portion). Benefits come from the $475,000 Gross Settlement Amount after attorneys' fees, costs, and service awards.
Documented losses may receive higher compensation.
