The Fairchild Medical Center Pixel Settlement is open to roughly 1,000 people, and only those 1,000. Do I Qualify?

You may be eligible if:

• Fairchild Medical Center's own records identify you as a California citizen whose private information was disclosed to third parties through the Meta Pixel and related technologies on its web pages — a closed list of approximately 1,000 individuals, not an open claim anyone can make
• You were mailed a notice on or after July 30, 2026; the notice is the clearest sign of class membership, and it carries the LoginID and PIN needed to file online as well as the postcard enrollment code for the privacy service
• Eligible states: California only, and in practice only the people already on the administrator's list for this one hospital in Yreka, Siskiyou County — not California residents generally, and not everyone who happened to browse the hospital's website
• You are not in an excluded group: the judge and the judge's staff and immediate family, the hospital's officers and directors together with its agents, affiliates, subsidiaries, parents, successors and predecessors, anyone who validly opts out, the legal representatives or assigns of any excluded person, and Class Counsel are all outside the class
• You submit a valid claim form for the cash by November 2, 2026 — the privacy service needs no claim form at all

Two different things are being asked of you, and only one of them is real paperwork. Documentation: none. The notice and the claim form both state that you do not have to provide any proof or explanation to claim the $25 — no receipts, no records, nothing about a loss. The claim form asks for your name and contact information, a checked box for the one-time $25.00 payment, one payment method and a signature made on penalty of perjury. Credentials: yes, for the online route. The claim portal only opens with the LoginID and PIN printed on the notice that was mailed to class members on July 30, 2026, so someone who never received the notice or discarded it cannot start the online form. That administrator-issued credential is what this page counts as proof. The way around it is an envelope, not a workaround: the printable Claim Form marks its Login ID field “if known,” so a completed and signed paper form can be mailed — or emailed as an image — without it. What Happened?

Lori Delgado sued Siskiyou Hospital, Inc. d/b/a Fairchild Medical Center on July 22, 2024 in the Superior Court of California, County of Siskiyou, Case No. 24CV08548. The complaint alleged that the hospital let third-party tracking technologies, including the Meta Pixel, collect and disclose patients' personally identifiable information from its web pages without permission. The claims pleaded were negligence, breach of implied contract, unjust enrichment, breach of fiduciary duty, invasion of privacy, invasion of privacy under the California Constitution, violation of the California Invasion of Privacy Act, violation of the Confidentiality of Medical Information Act, violation of the Comprehensive Computer Data Access and Fraud Act, and violation of California's Unfair Competition Law.

Fairchild Medical Center demurred in October 2024 and again in December 2024. After a hearing on January 23, 2025, the court overruled the demurrer as to all counts on April 2, 2025. The parties took discovery and then negotiated from November 2025 into February 2026, producing this agreement. Fairchild Medical Center denies any wrongdoing whatsoever, says it believes it would have prevailed at summary judgment or trial, and the agreement states expressly that it is a compromise and not evidence of liability or any admission. No court has found that the hospital did anything unlawful.

The class here is unusually small and unusually specific: approximately 1,000 California citizens, identified from the hospital's own records, for a single community hospital in Yreka, Siskiyou County. Simpluris is the settlement administrator. Notices went out July 30, 2026. The claim deadline is November 2, 2026; the deadlines to opt out or object are both September 28, 2026; and the final approval hearing is set for October 15, 2026 at 9:30 a.m. Pacific Time in Yreka. The court has not granted final approval and no payment date has been announced.

How Much Will I Actually Get?

The two benefits are cumulative, not an either-or pick. Paragraph 2.3.b.2 of the settlement agreement says enrollment in the privacy service “may be combined with a claim for cash payment on the same Claim Form,” and the claim form and settlement website both use the phrase “and/or.” Taking one does not cost you the other. CASH: a one-time payment the notice describes as expected to be $25.00. This is not a share of a common fund. Paragraph 2.2(d) obligates Fairchild Medical Center to pay an amount equal to $25.00 multiplied by the number of approved claims, by check or wire within 20 days of the effective date, so the per-person figure is not cut pro rata no matter how many people file. You do not have to provide any proof or explanation to claim it. PRIVACY SERVICE: one year of CyEx Privacy Shield Pro, which every class member is entitled to automatically. Enrollment codes were already sent by postcard, and instructions for redeeming them are due within 20 days of the effective date. The agreement lists Dark Web Watchlist, VPN, password scanning, private search, password defense, digital vault and data broker opt-out services, with access to a fraud resolution agent. It is not claimed on the claim form, so filing a claim is only about the cash. THE DOLLAR CAPS IN THIS SETTLEMENT DO NOT CAP CLASS MEMBERS: attorneys' fees and costs of up to $100,000 and a service award of up to $2,500 for the class representative are both caps on what the lawyers and the named plaintiff can be paid, and Fairchild Medical Center pays them, plus all notice and administration costs, separately from the class benefits. None of it is deducted from anyone's $25. There is no aggregate settlement cap at all. TIMING AND RISK: nothing is paid until the settlement is effective, which the agreement defines as ten days after all the conditions in Paragraph 9.1 are met — after the final approval hearing set for October 15, 2026 and after any appeal window has run. Fairchild Medical Center may terminate the settlement outright if more than 75 class members opt out, which is about 7.5 percent of a class of roughly 1,000, and either side may terminate if the court refuses approval in any material respect. A check that has not cleared within 120 days of issuance forfeits the cash benefit.
